Synagogue Sponsor a Kiddush

Kiddush Sponsor Form

Chabad of Agoura Hills offers a beautiful sit down lunch Kiddush after services every Shabbat and Holiday afternoon. To help defray the costs, we appeal to our congregants and friends to sponsor the Kiddush in honor of a special occasion such as a birthday, anniversary, yahrtzeit, etc.
I would like to co-sponsor the kiddush for the following dates:

If you would like to sponsor a Kiddush, please select the week you would like to sponsor. Please let the Rabbi know the occasion for the sponsorship so that he could also announce in Shul.
